Sending money with your request is preferable.
It is acceptable but not necessary to use the New York State Genealogy Application form to request a record. The town does not have an official form of its own.

The Town Clerk's Office adheres to N.Y.S. Vital Records issuing procedures for vital records and and genealogy requests. For security purposes a photocopy of driver's license or some form of identification is required with requests.

The record sent to you may be a photocopy of the original entry or abstracted information filled in on a standard form.

It usually takes several days on average to answer a written request for a record.
