The search fee is $11 for a 3-5 year search
and $10 per hour for each additional year.

You may request vital records in person, by mail, or by fax. You may request an information packet only by phone (Tel # 845-451-4203). Please call for specific details in order for the office to accept a faxed request

SASE is not necessary. Please send money order or certified bank check with your request. The $11 fee is for the search whether information is found or not.

Requests must be made using the city's official form or the New York State Genealogy Application form (they are identical).

The record sent to you is abstracted information filled in on a standard form for records 1881-1923. A photocopy of the original entry is sent from records 1923-present. To obtain a record after 1923 (or within 75 years of death/marriage or 50 years of birth), you must present proof that you are a direct descendant of the person whose record you request.

It usually takes 8 weeks to answer a request for a record because of high volume.
